The Foundation's program, New Jersey Health Initiatives, was designed to support projects in New Jersey that develop new approaches to resolving the state's health care needs, focusing on the Foundation's goal areas.This hospital-based project, under the Foundation's New Jersey Health Initiatives program, will organize an interdisciplinary team of medical and social service professionals to develop comprehensive care plans for individuals and families facing end-of-life decisions. Special emphasis is placed on pain and symptom management, as well as spiritual and emotional support. Educating physicians and other health care providers in the practice of palliative care medicine will be an important component of this project.
