Improving chronic disease management in correctional institutions

The U.S. incarceration rate has increased nearly 600 percent from 1970, with more than 2 million persons in prisons as of February 2000. Prevalence rates for many chronic conditions, including asthma, diabetes, and AIDS, are often higher in inmates than in the general population. However, relatively few state correction departments have practice guidelines for chronic care. The purpose of this project is to document the use of practice guidelines and the quality of care provided for chronic conditions in correctional institutions in Georgia and Michigan. The project is developing and testing a tracking model to validate the use of standardized encounter forms as measures of the degree of adherence to approved clinical practice guidelines. The project will be considered successful if: (1) correctional health care systems adopt the standardized information system process; (2) the quality of health care services is documented for selected chronic conditions; and (3) an accurate and complete database is developed for use in ongoing quality improvement assessments. If successful, the long-term goal is to develop an informational system for outcomes measurement for correctional institutions nationwide.
