Tracking and understanding national improvements in chronic illness care

According to a recent landmark study, just over half of recommended care is provided to adults with chronic conditions. The use of evidence-based care management processes (CMPs) defined by the Chronic Care Model holds great promise to improve the quality of care for the prevention and management of chronic disease. This grant project grows out of results of the first RWJF-funded National Study of Physician Organizations in 2000-2001 (NSPO I) which found that the use of CMPs for asthma, congestive heart failure, depression, and diabetes in large physician organizations was fairly low overall, but more common in the presence of several key drivers including performance measurement and reporting, external incentives, and information technology capacity. This study (NSPO II) would repeat the national survey in 2005 to assess the current status of CMPs, as well as the spread and influence of the factors believed to motivate and facilitate their use. The project also would explore efficient strategies for examining links between the use of CMPs and clinical outcomes and assessing CMP usage and drivers in smaller physician practices. The project will be considered successful if it produces and disseminates results that assess national progress on key indicators of the Foundation's quality strategy. Co-funding will be provided by the Agency for Healthcare Research and Quality, the California Health Care Foundation, and the Commonwealth Fund.
