Consulting to perform the initial six-month phase of work for RWJF's Aligning Forces for Quality program

This grant provides support for a consultant to perform the initial six-month phase of work for the Foundation's Regional Market Pilot. This Regional Market Pilot is a set of quality improvement activities targeted in specific communities and is designed to help those communities align quality improvement efforts across their respective care markets. The consultant will: (1) assist the Foundation in the investigation of probable pilot communities for this project; (2) prepare a written report regarding this investigation; (3) assist the Foundation in final selection of the pilot communities; (4) help the Foundation select an advisory committee; and (5) assess potential technical assistance needs for the pilot communities. The consultant will be critical to the success of this pilot project, and the pilot project, in turn, will be critical to the success of the Foundation's strategic objective to accelerate performance improvement on nationally adopted measures of outpatient chronic care quality through local market demonstrations.
