This grant provides the Center for Health Improvement (CHI) with technical assistance resources to support the initial four sites selected under the Foundation's Aligning Forces for Quality: The Regional Market Pilot. This national program will help communities develop and align three key drivers of quality improvement--measurement and public reporting, provider improvement and consumer engagement. This program is a key element of the Foundation's strategic objective "to accelerate performance improvement on nationally adopted measures of outpatient chronic care quality through local market demonstrations."
CHI will use these resources to provide technical assistance to the program in year one by: (1) arranging two net teleconference calls for grantees related to performance measurement and public reporting--since program grantees are being asked to measure and report community-wide health performance information at the physician organization or individual physician level, the purpose of the teleconferences will be to share different market models for collecting clinical performance information at the physician level in each of the markets; and (2) forming and managing a consumer engagement group learning process--due to the lack of existing evidence around consumer engagement strategies, this group learning process will maximize participants' knowledge of promising practices and allow for shared feedback of experimental strategies across communities.
