The Foundation's program, Catalyzing a Health Information Environment to Support Individuals and Families, was designed to expand the personal health records vision and encourage the market to develop a variety of products that will meet the diverse needs of patients. This project seeks to develop a PHR uniquely suited to meet the needs of children with chronic illnesses (specifically cystic fibrosis). Children are considered to be unique as they have less control over their medication administration (e.g., a care giver or parent gives them their medication) and have different dosing requirements based on body size/weight and age. The team envisions their PHR connecting disjoint care teams (school nurse, parents, primary caregiver) through their application to deliver continuous care to children.
