The Foundation's program, New Jersey Health Initiatives, was designed to support projects in New Jersey that develop new approaches to resolving the state's health care needs, focusing on the Foundation's goal areas.This grant enables Project H.O.P.E. to implement an Integrated Mental Health and Substance Abuse Treatment Model for homeless individuals suffering from mental illness and addiction. The model includes outreach and engagement, extensive case management, integrated treatment and psychotropic medications. Reducing obstacles with this approach will increase accessibility and improve the likelihood for overcoming homelessness.
