The Foundation's initiative, Aligning Forces for Quality: The Regional Market Place was designed to help communities across the country align health care forces to improve the quality of health care. Building on George Washington University's (GWUs) expertise serving as the National Program Office (NPO) for Aligning Forces for Quality (AF4Q) since 2008, through this grant they will also serve a second year as the NPO for the AF4Q Population Health Project (formerly called AF4Q Mobilizing Action Toward Community Health program) beginning on October 1, 2012. During the first of two years as the AF4Q Population Health Project NPO, GWU's activities focused on three major domains of activity: (1) planning, implementing, and monitoring technical assistance (TA); (2) supporting and tracking the progress of up to 5 AF4Q MATCH grantees' work; and (3) serving as a liaison to RWJF and other Population Health Project partners. In year two, the NPO will continue to identify appropriate TA providers, execute and manage sub-contracts with the TA providers, develop and implement TA plans and a monitoring system for the program's progress, ensure that all activities are aligned with broader AF4Q goals, and communicate the progress, challenges, and successes to RWJF. If there is a common TA need related to the Alliance-selected health behaviors, social/economic factors, or systems and policy change approaches, the NPO will organize global training and TA. The NPO will organize a meeting of grantees. In addition, the NPO will ensure collaboration between the University of Wisconsin Public Health Institute, Community Catalyst as the Roadmaps Community Grants program NPO, RWJF, and other key stakeholders. Using the monitoring and tracking system, the NPO will capture and share stories and lessons learned from AF4Q Population Health Project through the public AF4Q website and other social media outlets. As this is a two-year project, the NPO will work with the grantee Alliances to make sure efforts to sustain the project activities are incorporated early on.
