Evaluating the RWJF initiative County Health Roadmaps, 2012-2017

The Foundation's initiative, Mobilizing Action Toward Community Health (MATCH), was designed to help communities translate the County Health Rankings into multisector action that addresses the social, economic, environmental and behavioral factors that affect health.This grant supports Mathematica Policy Research in conducting an evaluation of the County Health Rankings and Roadmaps (CHRR) initiative. The evaluation will focus on how and whether the four components of the Roadmaps to Health program are able to: (1) increase awareness that health is determined by multiple factors; (2) expand multisector community efforts to address multiple determinants of health; and (3) change policy, systems and the environment for community health improvement. To assess these outcomes, Mathematica will examine each of the four program components, individually and combined, using a mixed-method evaluation design. Data-collection deliverables will include key-informant interviews with stakeholders, document reviews, and a feasibility study to survey community leaders on awareness. Data analysis deliverables will consist of content analyses of interviews and document reviews, quantitative analyses of feasibility study data, secondary analyses of CHRR datasets, a social network analysis of a sample of community grantee coalitions, incorporation of media-impact findings, and a cross component analysis of the four major program areas. Dissemination deliverables will be quarterly and annual updates in a format that can be shared with general audiences, an in-depth final report, presentations at internal and external meetings, publication in academic journals, and possibly a public-use dataset for inclusion in the Robert Wood Johnson Foundation (RWJF) Health and Medical Care Archive. Mathematica will conduct the evaluation in consultation with RWJF, the University of Wisconsin Population Health Institute, and the CHRR national program office to allow for formative feedback in both program and evaluation implementation.
