The Foundation's Roadmaps to Health Community grants initiative was designed to identify, support, and disseminate models of cross-sector coalitions working to improve health by taking action to address the social and economic determinants of health, such as education, employment, income, community safety, and family and social support.The Institute for Native Pacific Education and Culture (INPEACE) seeks to collaborate with the Hawaii Department of Education and other partners to incorporate the community schools concept at schools in the Nanakuli-Waianae Complex, a rural area serving the largest population of Native Hawaiian and economically disadvantaged students in the state. Ke Ala Hanau Moku (KAHM), a multi-sector, community-based coalition, will guide this project. The project, one of two community school efforts in Hawaii will serve as an example upon which the state plans to roll out similar reforms state-wide. INPEACE and KAHM will conduct outreach to engage and empower the community, will develop a coalition infrastructure to implement the community schools framework in at least three local schools, will establish a community school coordinator at each participating school, and will document progress to promote the adoption of this approach across the state. No RWJF dollars will be used for lobbying. Deliverables include collaborating with the Department of Education, implementation of three community school pilots, hosting community meetings, engaging local service providers to work in a coordinated manner in the schools, and hosting a conference focused on sustaining the community schools approach in Hawaii.
